The form given to the customer is a carbon copy of the original that Volks keeps, so can sometimes be hard to read. As you can see, the details of the order are written in by the Volks staff member with pen; they are not printed on, with the exception of body parts that only have one available option, such as the SD13 boy body, or the MSD arms.

Be aware that the SD form has a picture of a girl on it whether you order a girl or a boy. (Note: we have heard of rare occasions of SD forms with boy pictures, but it's usually a girl.)
